Where Care Meets Community: Discover Hart Community Homes Heal, educate and empower at-risk youth to become healthy, productive and self sufficient adults in the community. We provide the necessary continuum of care to youth in, and emancipating out of, the foster care system in order to eliminate homeless youth and adults in the community. Youth “age-out” of the foster care system at the age of 18 and often lack adequate independent living skills. At Hart Community Homes, we believe that every child deserves a chance to succeed, no matter the circumstances they face. Founded in 1996, our organization sought out to support Orange County's foster care youth. Today, we proudly operate two state-licensed residential treatment homes in Orange, CA, catering to boys aged 13-18, and a transitional home, housing young adults age 18 - 25. We recognize that the work we do goes beyond just providing a roof over their heads. From the moment these individuals arrive at one of our homes until well after they are successfully emancipated into the community, they are provided with an array of services and support.
